Right now, We are seeking for a proactive CDE with strong clinical skills on diabetes management, to promote diabetes-related literacy that will positively impact a self-care lifestyle through treatment and advocacy, and boost well-being in our culturally diverse population.
What you will do in this function:
As a driven and change-inspiring Certified Diabetes Educator, you will promote healthy eating habits through individual nutritional assessment, diagnostic therapy, and counseling services, to educate patients and their families with or at risk for diabetes on self-management. ¬Collaborating with multidisciplinary teams, you will perform in-depth motivational interviews to identify prospective and fully-fledge diabetes; develop a treatment program located on evidence-located procedures and regulatory requirements and monitor adherence. You will also train and educate nursing staff; attend meetings, in-service training, and outreach activities; and participate in quality assurance/improvement monitoring and reporting initiatives.
To qualify, you must possess:
   » Bachelor's degree from an accredited institution or foreign equivalent; Master's degree required by 2024
   » A minimum of 2 years' clinical practice as a Registered Dietitian in an outpatient/community setting; 1-year practice as a CDE preferred
   » Completion of an ACEND accredited/approved Supervised Practice Program in Dietetics
   » Certification with New York State as a Certified Dietitian/Nutritionist (CDN)
   » Registration with the Commission on Dietetic Registration (CDR)
   » Board certification for assignments in specialty areas including Nutrition Support, Renal, Pediatrics, Geriatrics, Oncology - preferred
   » Excellent oral, written, and interpersonal communication skills
   » General expertise of nutrient analysis, word processing and spreadsheet software
